Preventive maintenance schedule template for facilities teams

A preventive maintenance schedule turns an asset list into work that has an owner and a due date. Use this planning template for building equipment when you need a practical annual plan and monthly checklists without setting up a full maintenance management system.

Start with an asset register

List the equipment you are responsible for before adding tasks. Give each asset a consistent identifier and location so a technician can find it. Separate similar assets if their instructions, usage or service history differ.

  • Asset: name, identifier, location and relevant equipment details.
  • Task: the work to plan, with a reference to approved instructions where needed.
  • Interval: frequency chosen from manufacturer guidance and your maintenance program.
  • Due date: the next planned occurrence and the basis for scheduling it.
  • Owner: the person or team responsible for arranging the work.
  • Completion record: date performed, notes and follow-up work in your chosen record system.

Keep completion dates and findings in the maintenance records you already use. A planned date is not evidence that the work was completed.

Example: distinguish the task from the asset

An illustrative asset entry might identify “air handling unit AHU-01, roof mechanical area.” The task entry should point to the specific maintenance instruction rather than saying only “check HVAC.” Choose the interval from the equipment documentation and your program; this example does not prescribe a service frequency.

Build a 12-month view

Review the annual calendar for clusters of work, access restrictions and planned shutdowns. Confirm that the people responsible have the right instructions and resources. Then use monthly checklists to hand over the planned work and calendar exports to place due dates in your scheduling system.

Keep the plan current

Review the schedule when equipment is replaced, instructions change or recurring faults suggest the plan needs attention. Adjust future work through your maintenance process and retain the completed-work records separately. A simple schedule helps organize work, but it does not replace manufacturer instructions or qualified decisions about equipment safety.
