What to record on the attendance sheet
Put the identifying details above the names. An attendance list without a date or topic is difficult to match to a meeting. Keep the topic specific enough to distinguish this discussion from another session.
- Date and location: when and where the meeting took place.
- Topic: the safety subject and any site-specific issue discussed.
- Facilitator: the person who led the discussion.
- Participants: printed names and signatures.
- Meeting notes: questions, observations and agreed follow-up actions.

Example: make the record useful after the meeting
For an illustrative housekeeping discussion, record the work area and the issue the crew raised, rather than only writing “housekeeping.” If someone agrees to arrange storage for loose materials, record the action and owner. The attendance sheet and notes can then be understood by someone who was not in the meeting.
Run the discussion and collect attendance

Attendance and training are different records
A signature records attendance; it does not by itself demonstrate competency or completion of required training. Use the talk and sign-in record as part of your safety communication process, alongside any training and assessment your work requires.
